People on disability benefits rely heavily on their family doctors. They need them to fill out forms, develop treatment plans and communicate with disability benefit providers. However, as more doctors retire and fewer medical school graduates open family practices, many people on disability benefits are now faced with the challenge of navigating their claim without a family doctor.
This issue is especially problematic to the community because most disability benefits providers require claimants to be under the regular care of a doctor. Without one, their benefits could be cut-off at any moment. On top of that, people who are applying for disability benefits need a doctor to fill out medical report forms.
That is why Resolute Legal compiled a list of virtual clinics that offer services for filling out disability medical report forms. That way, people who don’t have access to a family doctor can still get their forms filled out. And people who are already on disability benefits can have access to regular care.
List of Online Clinics that Fill Out Disability Medical Report Forms
The following is a list of virtual clinics and doctors who fill out disability medical report forms. Keep in mind, however, if you don’t have employer or insurer coverage, fees may apply since many virtual services aren’t covered by provincial health plans.
- Tia Health: Completes disability medical report forms for $70. They operate across Canada.
- Rocket Doctor: Rocket Doctor has no policy on the completion of disability forms. Some doctors will complete disability forms, while others will not. However, Rocket Doctor says they “will match patients presenting with medical requirements, including forms, to physicians willing to provide that level of care.” They operate in Ontario, Alberta and British Columbia.
- Medicuro Online Clinic: Fills out Disability Tax Credit forms and WorkplaceNL forms. These services are available to residents in Newfoundland and Labrador.
- WELL Health Company: Completes disability forms for people in Ontario and Alberta.
- Maple: some physicians at Maple may fill out disability medical report forms. However, they advise patients to provide as much detail in the initial consultation request to help ensure they can match them with a doctor that is willing to fill it out. Maple operates across Canada.
- ClaimDocs: works with personal injury and disability law firms to enable physician access to clients without a family doctor. ClaimDocs will complete all standard insurance and workers’ compensation claim applications as well as federal and provincial disability program applications. Note: in order to use this service, you have to work with a law firm that is enrolled with ClaimDocs.

People on disability benefits should only use these virtual services if they have exhausted every other possible option. Meaning they have looked for a family doctor without any success and asked all other medical practitioners they are under the care of, such as specialists and nurse practitioners, to fill out their disability medical report forms.

They prefer when your forms are filled out by a doctor who knows you well and can accurately describe your symptoms and limitations.
